Revisional Cosmetic Eyelid Surgery
The eyes can be the most distinctive feature of a person's face. The goal of good cosmetic surgery is first and foremost to preserve health and function, followed by an improved and natural-looking result. Too often cosmetic eyelid surgery results in a "surgical look" or visual problems.
Revisional cosmetic eyelid surgery (or revision blepharoplasty) can address poor results from the primary surgery. However, this is one of the most difficult areas of oculoplastic surgery to master. Revision surgery is more complex than primary surgery because the surgeon must operate on planes with scarred planes with distorted anatomy. This means that there are unknown factors in the surgery that can only be identified once. This makes the surgery unpredictable. In addition, it is often difficult to meet a patient's expectations from surgery, as there is a desire to recreate what existed before the first "bad result" procedure.
The goal of revision eyelid surgery is to make the patient look close to how they looked before the initial procedure, but with the caveat that this must be done safely and with realistic expectations. Op. Dr. Aytek Coşar has developed himself in revision eyelid surgery and has performed many successful surgeries in this field.
Cosmetic surgery is rapidly evolving into a specialized medical discipline. There are numerous subspecialists in this field with different training backgrounds from general, facial, nasal, ophthalmic and dermatologic plastic surgeons. This evolution in cosmetic surgery reflects the general trend in medicine and surgery towards the development of super-specialists. Today, cosmetic eyelid surgery, especially in the case of complex revision surgery, should be performed by ophthalmic plastic surgeons (board-certified ophthalmologists with additional subspecialty training in cosmetic surgery of the eyelids) to ensure the best possible outcome for the patient.
Revision Surgery After Cosmetic Eyelid Surgery
The goal of primary cosmetic eyelid surgery is to reverse the signs of aging around your eyes. A proper blepharoplasty procedure will make you look refreshed, rested and younger. However, in some cases, the primary surgery does not go as the patient planned (due to poor technique of the surgeon, complications from the procedure, expectations for the surgery not being met, etc.) and the results can actually age the patient or change the shape of the patient's eyes in an undesirable way. In this case, revision eyelid surgery may be required to restore the aesthetics and/or function of the eyelids. Since revision surgeries are always more complex than primary surgeries, it is important for patients to find an oculoplastic surgeon with extensive experience in revision surgery, such as Op.Dr. Aytek Coşar.
Reasons for Revision Eyelid Surgery
Patients seek blepharoplasty to eliminate the appearance of aging and fatigue around the eyes. However, overcorrection or undercorrection resulting from a blepharoplasty (for problems such as under-eye bags or hollows, drooping or loose upper eyelids, excess skin around the eyes, fine wrinkles, excess fat deposits that appear as puffiness on the eyelids, or drooping lower eyelids) can lead to aesthetic and/or functional problems for the patient that require revisional surgery.
The most common reasons patients seek revision eyelid surgery are:
Lower Lid Retraction - the lower eyelid is pulled down so that the lower lid remains lower and the shape of the eye changes.
The outer corner of the eye (lateral canthus) was cut during the primary surgery, so the eye looks and feels different.
Eyes cannot be closed (too much tissue removed from the upper lid).
Fat grafting causes lumps, swelling and contour problems.
Op. Dr. Aytek Coşar performs the following revisional surgeries in addition to revisional cosmetic eyelid surgery (or revisional blepharoplasty):
Revisional brow lift
Revisional mid-facelift
Revisional canthoplasty (surgery on the outer or inner corners of the eyelids)
Revision ptosis repair
Revision eyelid fat grafting

Bottom Cover Retraction
The most undesirable complication of lower lid blepharoplasty is lower lid retraction, where the lower eyelid is pulled down. The goal of correcting lower lid retraction is to restore the natural lower eyelid shape so that your eyes look more alive.

Fat Graft Repair
Unfortunately, under-eye fat grafting can lead to lumps, swelling and contour problems. This is a very complicated complication to correct. Dr. Massry has pioneered a procedure that allows him to remove the fat lumps and bumps through an incision inside the lower eyelid so that there is no visible scarring.
